What information does the Application obtain and how is it used?
User-provided information
The Application acquires the information you supply when you download and register the Application. Registration with the Service Provider is not mandatory. However, you might not be able to use some of the features offered by the Application unless you register.
The Service Provider may also use the information you provide to contact you from time to time with important information, required notices, and marketing promotions.
Automatically collected information
The Application may collect certain information automatically, including, but not limited to, the type of mobile device you use, your mobile device’s unique device ID, its IP address, your mobile operating system, the type of mobile Internet browsers you use, and information about how you use the Application.
Does the Application collect precise real-time location information?
The Application does not gather precise information about the location of your mobile device.
Do third parties see or have access to information?
Only aggregated, anonymized data is periodically transmitted to external services to help the Service Provider improve the Application and its service. The Service Provider may share your information with third parties in the ways described in this privacy statement.

The Service Provider may disclose user-provided and automatically collected information:
- As required by law, such as to comply with a subpoena or similar legal process.
- When they believe in good faith that disclosure is necessary to protect their rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.
- To trusted service providers who work on their behalf, have no independent use of the information disclosed to them, and have agreed to adhere to the rules set forth in this privacy statement.
What are my opt-out rights?
You can halt all collection of information by the Application by uninstalling it. You may use the standard uninstall process available on your mobile device or through the mobile application marketplace or network.
Data retention and managing your information
The Service Provider will retain user-provided data for as long as you use the Application and for a reasonable time thereafter. The Service Provider will retain automatically collected information for up to 24 months and may then store it in aggregate.
To request deletion of user-provided data submitted through the Application, contact privacy@intake.now. To delete your account and the data linked to it, see Account & Data Deletion. The Service Provider will respond within a reasonable time. Some or all of the user-provided data may be required for the Application to function properly.
Children
The Application does not address anyone under the age of 13. The Service Provider does not knowingly solicit, market to, or collect personally identifiable information from children under 13.
If the Service Provider discovers that a child under 13 has provided personal information, it will be deleted from its servers immediately. If you are a parent or guardian and are aware that your child has provided personal information, please contact privacy@intake.now so the Service Provider can take the necessary action.
Security
The Service Provider is committed to safeguarding the confidentiality of your information. Physical, electronic, and procedural safeguards are used to protect the information processed and maintained. Access is limited to authorized employees and contractors who need the information to operate, develop, or improve the Application. Although the Service Provider endeavors to provide reasonable security, no security system can prevent every potential breach.
Emails to healthcare practices
The Service Provider occasionally sends an email to the general address of healthcare practices to introduce intake.now. For this, the name and general email address of the practice are used, as published on the practice’s website. This is based on the legitimate interest of the Service Provider in informing healthcare professionals about a service relevant to their practice.
The emails are sent through Lettermint, which processes the data on behalf of the Service Provider. It is recorded whether an email was delivered, whether it bounced, and whether it was reported as spam. These figures are used to keep the list accurate and to compare versions of a text, not to build a profile of any person.
Every email contains a link to unsubscribe. Your address is then removed from the list immediately. What was sent to an address, and when someone unsubscribed, is kept so that the Service Provider can demonstrate that this choice is respected.

Changes
This Privacy Policy may be updated from time to time for any reason. The Service Provider will notify you of changes by updating this page. You are advised to review this Privacy Policy regularly, as continued use is deemed approval of all changes.
Your consent
By using the Application, you consent to the Service Provider processing your information as described in this Privacy Policy, now and as amended. “Processing” means using cookies on a computer or handheld device, or using or handling information in any way, including collecting, storing, deleting, using, combining, and disclosing information.
